Acceptable Documents – US Background Check

This document outlines the various acceptable documents for HireRight. HireRight requires that documents provided by candidates must be authentic. Documents must not be handwritten and must include the candidate's name, the source name (with additional explanatory association if applicable), and date information. Documents that do not meet these requirements will be considered unacceptable.

If you are currently employed (CEV), please provide a recently issued document from your company, such as your most recent pay stub. Please note that the document must clearly show that you are currently employed. Annual documents (such as W2 or 1099) cannot be used to prove the current employment status end date.

Acceptable Employment Verification Documents

  • Paycheck Stubs
  • W-2 Form
    • Printed W-2s from TurboTax, H&R Block, etc. are not accepted.
  • W-2s Wage and Tax Statements from the IRS
  • W-2 Summary Statement IT-2
  • IRS Tax Records or W-2 Form Copy | Internal Revenue Service (irs.gov)
  • Form 944 Federal Tax Return (requires third-party preparation and signature page)
  • Form 1042-S
  • Business Checks
  • Certificate of Salary Status
  • Commission Checks (prepared by employer)
  • Copies of Online Paycheck Stubs
  • Civilian Leave and Earning Statements
  • Direct Deposit Slips Issued from Employer
  • Employee Profile Printouts
  • EODS/EDOS/EDDS IW-2 Forms
  • Itemized Commissions Sales Check Stub
  • Itemized Statements of Earnings from SSA (Social Security Administration)
  • Itemized W-2 Form
  • Labor Law Documents (must be signed and dated by employer)
  • Personnel Change Notification Forms (New Hire Reports, Termination Reports, Exit Notices)
  • Severance Information Sheet
  • Statements of Earnings
  • UBER Payment Statements
  • W-2 CG Form (for US Coast Guard)
  • Work History Report from SSA (Social Security Administration)

Acceptable Documents for Self-Employed Individuals Only

  • Form 1040 (requires third-party preparation and signature page)
  • Form 1042-S
  • Form 1065 B (requires third-party preparation and signature page)
  • Form 1099 Misc. (if only company name is provided, proof of association between the company and the applicant is required)
  • Form 1099 NEC (as described above)
  • Form 1099K (as described above)
  • Articles of Incorporation/Organization Certificate/Formation Certificate filed with government agency (must include stamp/seal/date)
  • Fictitious Names/DBA
  • Business Licenses
  • Commission Checks (prepared by employer)
  • Itemized Commissions Sales Check Stub
  • Itemized Earnings Statements from SSA (Social Security Administration) with employer name included
  • K-1 Shareholder Income, Deductions, Credits (with third-party signature)
  • NYC 202EZ Unincorporated Business Tax Return (requires third-party signature)
  • Sales Tax Permit
  • Schedule C (requires third-party preparation and signature page)
  • NGB22 Form

Acceptable Documents – Military and Government Employees

  • Civilian Leave and Earning Statements
  • DD214
  • Leave and Earnings Statements
  • Member 4 Military Documentation
  • NGB22 Form
  • Personnel Change Notification Forms (applicable only for hiring or termination, such as exit reports, termination notices)
    • Please note, for documents of this type to be accepted, the following must be clearly displayed: the complete form, applicant name, employer name, date, and official state seal or signature of authorized form signer.
